First of all is your license restricted or suspended? If it is suspended you will need to go through your state's Dept. of Transportation or Dept. of Motor Vehicles. Either of those offices can tell you the information you need and the steps you need follow.

We had to get ours from the school....get a form from the guidance department and fill it out and return it....

when I was in school I know you got them from your schools guidance councler (sp) but that was 12years ago... would think you could still get them there
