Did you think to remind her a few days before hand . . . and how long have you been dating.
I'd go ahead and mention something about, 'Lets go out tonight to such and such place for My birthday.' and see what she says. Give her the opportunity to make it up to her rather than just make her feel like crap.

Tellher, she should have remembered already. Ask her "do you know what date is today?" and see if that rings any bells, if it doesn't, she will have to make it up to you.
