I suppose it depends on how you dont want to die. There are a number of conditions with truely horrible symptoms where the mortality is as near as damn it 100% once you've got it - and there is nothing that medical science can do. Ebola and rabies are particularly nasty. Of the two I'd say that rabies is the worst because it lasts a bit longer.
Of the survivable conditions I think tetanus would be absolutely awful if you were in a country with poor medical services. In the western world, where they could keep you alive, Locked-In syndrome following a stroke. In this condition you are alert and conscious, can see and hear but cannot speak or move any part of your body - you are like a brain in a bottle - awful

I guess that depends on how you would qualify the worst. There's a "laughing disease" you can contract in the western Pacific, and it's 100% fatal (I guess dying laughing wouldn't be too bad, but if you get it, you're gonna die). They say pancreatic cancer is the worst because there's no cure and it's very painful. AIDS is a terrible disease because of the social stigma attached to it. How about leprosy? No one would want to be around you or touch you...Hmmm...tough choice.

First of all, HIV is not the worst ilness you can get. There are all kinds of meds for it and plenty of hope for a cure.
I gotta say Alzheimers or Parkinsons. It's one thing to die a slow death but another entirely to have your mind degenerate at the same time.

Define a "bad illness". With AIDS, you can end up suffering for a loooooonnng time - the prognosis is slow and painful. However, there are tons of other illnesses or infectious diseases where you die almost instantly.

I'd say Motor Neuron Disease - MND is a progressive neurological disorder that destroys motor neurones, the cells that control voluntary muscle activity such as speaking, walking, breathing, and swallowing. It can leave sufferers totally incapacitated and unable to carry out the simple day to day actions we all take forgranted.

Think about it, it kills you over 10-15 years and it does it by weakening your immune system. You actually get killed by numerous other things that would not hurt you normally. Worst part is you don't have any symptoms for 5 years making you a walking plague machine.

what can be worse than cardiovascular failure, complete failure of the nervous system leaving sufferers completely feeling pain perminantly whilst feeling nothing else, leading to complete mental madness. having the skin rot from every part of the body, and liver failure, and others. not mention it taking upto 20 years to finish you off! and all the while knowing it was because of unprotected intercourse with an infected individual!
nothing is worse.

cancer that dosent go away untill you are on deaths door and then does the same thing again all through your life!
The cancer then leaves you open to more illness.
