The stomach will not shrink. If one does not take food regularly you may end with gastritis problems. Although there is not enough food in the stomach, the gastric juices are still released into the stomach at regular intervals which will act on the stomach walls if there is no food for it to digest. The gases in the empty stomach will also give pain.
