Hemolysis occurs. Crenation occurs when the water has more salt than the body carries. This state is known as hypertonic. Distilled water is hypotonic.

The end product red in the water could more correctly be called hemolysis. crenation is the result of fluid leaving a red cell. This results in the cell having a prickly appearance. When added to distilled water the resulting osmotic forces would basically rip the membranes of a large portion of the rbc's. This is why a saline solution is used for IVs or washing red cells for testing or processing.Their hemoglobin would be released into the solution resulting in a red appearance. After a certain amount of centrifugation. The remaining red cells and membranes will be found on the bottom of the tube, and the liquid solution will appear red.

The RBC first become crenated, then rupture (lyse). Both processes are at work.
