2) Should have good problem/puzzle solving skills
3) Should have a good domain knowledge in which you are working. If you are a fresher, then have at least basic knowledge of the domain.

5) Knowledge of basic algorithms

depends on the job. For helpdesk, a basic knowledge of computers, Windows and networking will suffice. For systems engineering, you'll need to know more about computers, Windows and networking, as well as printing, office and other applications. For network engineering, you'll need indepth knowledge of networking, computer- and serverhardware and Operating Systems.
But these are all general guidelines, each company has their own requirements.

Certifications MAY be useful - that tends to vary depending on sub-field. But if you want to go to work for a consulting company, their consultants often don't have very many specific technical skills - actual coding may well take place offshore. What they do have are serious design & architectural ability.
