Anyone got ideas of what they might have?

If you are trying to say that they have self inflicted scratch marks in a set pattern on their skin they are most likely having an allergic reaction which is causing them to scratch their legs. Can they communicate verbally, or are you unable to understand them? I would be questioning the caretakers and the persons doctor(s), rather than an open forum of strangers. Older people have thinning skin and can be very sensitive to soaps and lotions and even the type of fabric which their clothing and/or bedclothes are made out of. If you are a family member, or a concerned member of the staff, I would definitely be checking with the other staff and doctors first, and then consider having some allergy tests done. It sounds as if this person is very uncomfortable, to be scratching themselves to excess. You may want to make sure that their fingernails and toenails are filed and kept clean; for fear of injury and infection. Good luck and have a good day!
