I think it means that you would have to kill them first. Like if someone said "Give me that book!" And you said "over my dead body!" You are saying to them that you are going to have to fight and kill me before you get it.

the only way it would happen is that the person speaking would have to die first. so, let's say you want someone's new blouse. she says 'over my dead body'. she's saying it's never going to happen unless she suddenly has a heart attack or you kill her.
