Cocaine is derived from the leaves of the coca bush, found mainly in Peru and Bolivia. After being extracted from the leaves, it is then processed with various chemicals. Cocaine usually comes in the form of a white odourless powder called cocaine hydroxide.

It's made from coca (not cocoa) leaves.
Native coffee farmers often chew on the raw leaves to keep themselves awake.
Cocaine is just a purified version of the chemicals in the leaves, but is often mixed with other substances to bulk it up.
