There are self-proclaimed countries that are not recognized as independent states by the U.S. (and majority of the world). For example - Republic of Ichkeria (Chechnya).

I think North Korea and Cuba are the only true countries that we have know ties with, although we do have an embassy in Havana.

Think Axis of Evil....and add Iran just have you had. I reviewed all my travel documents (which countries I could and could not go to) when I work in the middle east. I thought that it might be Syria but we have diplomatic ties.
I reviewed all the information with US embassies, consulates, missions, sections and interests listed or mentioned on the US State Department website, on www.firstgov.gov and the Dept of Defense website. I didn't find any other country with out diplomatic ties...
If anyone ID's one...please give me an FYI if you don't mind.

It used to be Libya, but we announced formal re-establishment of diplomatic relations this year (back in May), with an embassy scheduled to reopen in Tripoli next year.
Perhaps you're thinking of Taiwan? We can't formally recognize them as a separate nation-state without antagonizing the PRC, so we don't have formal diplomatic relations with them. Instead, we have a legal framework embodied in the Taiwan Relations Act of 1979.
