use whole milk and move the carafe up and down, don't just leave the steamer in the milk. moving it creates more foam.

Make sure the milk is cold first. Turn the steam on fairly high. be agressive (tip of the steam wand just below the surface) until it reaches about 110 degrees. Then set the wand deeper and into the side of the container and let it swirl the milk till it reaches temperature (usually 140 or 160.) Whack it against the counter a couple times and you have foam. Of course the most important part is Practice Practice Practice.

My friend used to put coffee, sugar and milk in a cup and mix it up and then put water or milk in and stir it all the time and it froths up.
