Depends, are you asking how to treat, or how effective an attack?


1. To treat: Have the person sit indian-style (legs crossed in front of them). Put your knee against their back for support, then gently lift them up by the armpits (not soft flesh, get a good grip first). This will help their testicles return to the normal position and prevent permanent damage, including sterility. If the testicles are swollen, see a doctor immediately.

2. As an attack. This attack is not for sparring or training of any kind. It is a defense against someone trying to kill or seriously injure you. It is generally accepted to be one of the dumbest ways to attack and should only be used by incredibly inaccurate people.
An attacker will often expect to be kneed in the groin, and therefore comes into a fight protecting that area. If you want to catch him off guard, it would be best to fake for the groin, wait till he goes to gaurd it, then do an eye gouge.
If you do not want to get in the range for an eye gouge, or your hands are not free, you should not waste an open groin with a knee to the testicles. If you have any accuracy, the much more effective maneuver is a toe-kick to the perineum. It is just behind the testicles, so if they are available, it is too, and, unlike the testicles, striking the perineum does not cause a rush of endorphins and adrenaline. It just causes pain. In fact, and kick that is well aimed and has enough power (very little if your aim is good) can knock a man unconscious with pain, allowing you to get away or permanently immobilize your target, depending on the needs of the situation.

I reiterate, no move such as this (knee to balls, perineum kick, eye gouge) should be used in day-to-day sparring or roughhousing, as they are health hazards.

n get away,is your best option.If your in close quarters,a knee to the balls followed up by you dropping to the ground with his forearm locked in your hands is the next step.Once your on your back proceed to wrap your feet around his neck and then twist and roll.He will drop to the ground once you apply the upwards,torquing pressure to his arm.This arm bar move can render anybody useless,since the attackers elbow has just snapped backwards,resulting in a tremendous amount of pain and disfigurement...tom science

I've had it done to me several times. It never worked right for some reason but it made me mad and I made the other person regret trying it.
It's not as easy as it sounds.
If you get one side or the other it isn't very painful.
I think there are more misses than direct hits.
If that's your only defensive move you're in big trouble.

A move that far too many martial artists are not able to defend against. Why? Because many train for tournament style fighting where they do not have to worry about it.
