Yes hair is to be covered. I wasn't aware of this verse in Corinthians, but I know that observant Jewish women cover their hair after marriage. Apparently, after marriage hair is not to be shown to anyone but the husband or immediate family. This is a strict observance of halakah and is based on Numbers 5:18 -- uncovering a woman's hair is shameful.
My understanding is that this is the reason that Orthodox Jewish women generally wear a wig either over their natural hair, or over a shaved head (to avoid showing their hair). However it was recently ruled that human hair wigs made of Indian Hindu hair is not Kosher, so a lot of Orthodox women are burning their human hair wigs, so we may see more of the traditional ancient style veil hair covering or just more bad synthetic wigs in the Orthodox community in the near future.

No, this was a human invention, one of those nifty little laws designed by a patriarchal church. The Bible quote is most likely taken out of context; the tradition is probably borrowed from the Jewish yarmulke tradition, and modified to just address women, and also may be a reflection of the modest dress of women, in ancient times and continuing today, in the region where Christianity originated. However, it became a sign of respect among Christians to keep the female head covered during mass. When you think about it, too, there could be basic hygenic reasons for this, especially in ancient times when things like lice were a lot more common, and people wanted to protect themselves and others when gathered in close quarters to worship. If anything, the Bible verse probably refers more to ancient Hebrew cultural convention more than religious doctrine.

There is an interpretation that sees the woman's hair as a temptation to the fallen angels. But that requires a study of Genesis 6 and Jude.

The beginning of that verse says "every woman who prays or prophesies . . . " and if you read further down (11:14-16) it says that "long hair is given to her as a cover."
