In my church, baptism is never repeated - even if you were baptized my members of another denomination. If you were baptized, you still are, no matter what you've done.
Most demoninations will have something you can do to show that you are rejoining the churchl In some denominations, confession is also available (not just catholic) upon request, if that would help you in your process of redemption.

I was baptized at the age of 7. I strayed in my teens and early 20's, but always came back during times of trouble. Once I had my first child, I was sitting in the nursery thinking of all that my daughter had (clothes, food, shelter) and God asked me, "What about her soul?" I know you've heard of that still small voice. I heard it. Well, I got my butt into church for sure! When my husband went forth to be baptized, with as much straying as I did in my younger years, I wanted to make things right and be baptized with him. I know I didn't have to, but I did it anyway. I'm pretty sure the Lord understands.

You can be baptized only once, even if you had strayed away from the Lord.
Most baptisms are performed by priests / pastors. However, any Christian can perform a baptism on another person--no matter what their faith or what Christian sect that other person belongs to--by pouring water on him/her while saying, "I baptize you in the name of the Father, and of the Son, and of the Holy Spirit. Amen."
You cannot perform a baptism on yourself.

You don't need baptized again. God enters you the first time and never leaves. He does it right the first time around.

well that's up to you and how you feel no one can tell you that you have to and no one can tell you, you cant you have to make the choose and for the pastor thing that's up to you also, but first you have to ask God to for give you of your sin's and the rest is up to you
