The children are born that way,in the past the doctors transformed(surgery) the children to be girls,because the size of the penis was very small. Now doctors wait to perform the operation till a dna test is done to determine what sex the child is.

shemales, or ladyboys, are usualy transexual males that have breast augmentation surgery, for example.
they are usually not born with it. while it's possible to be born with a penis and develop breasts, these people are usually not shemales.
women with penises can also be born this way, but they are usually just women with enlarged clitorises or other genital developmental malformation. they are not shemales.
