I don't know the percentage, but here are stories of women who have died from so-called "safe and legal" abortions:

Abortion is nearly four times MORE dangerous than childbirth:
http://www.afterabortion.info/news/abortiondeaths.html
Childbirth is a natural process. Abortion is completely unnatural. The cervix, tightly closed to protect the baby, must be forced open, often resulting in tears and permanent damage. Then, sharp instruments are inserted. Abortion is a "blind procedure," meaning the abortionist cannot see what he is doing. He may permanently damage reproductive organs or even the bowel or bladder. The International Planned Parenthood Federation estimates 19 million women worldwide will have an unsafe abortion in 2006; a similar number took this risky step last year - 70,000 died. This accounts for 13 per cent of the 500,000 maternal deaths each year.

That's a tough statistic to find since the Planned Parenthood groups don't want you to know about THAT part of the process. I would guess that in the US, the number is fairly small, but I don't think that is the bigger picture.
Although death from any surgical procedure is tragic, what is more tragic is the emotional damage that occurs later in life to those who abort their babies. That number is far greater and much easier to document.

It is pretty close to 0%, but the percent of women under the age of 18 who have a baby and die in child birth is much much higher.

I don't know the numbers but isn't 1 too many? Legal, safe abortions should be available in every state.
