Sevice request is any change made to services like if it is mobile or intenet- you change plan or add additional features. Change request would be related to your personal info details for a service that you are using like contact into etc

What are you referring to? That sounds like something out of the ITIL process.

If that's the case, then a service request is a request for a move, add, delete, etc., for hardware or software. A change request is a higher-level process that typically doesn't involve the end user directly. Usually a change is referring to a server or system that may require downtime or loss in service temporarily requiring management approval.

So it would be better for you to associate one to the end user and the other to the enterprise side of things...
