Tofu (also known as soybean curd) is a soft, cheese-like food made by curdling fresh hot soymilk with a coagulant. Then the curds are pressed and cut into blocks (or something like that). Studies show tofu is actually healthy for you.

Tofu, also called doufu (often in Chinese recipes) or bean curd (literal translation), is a food of Chinese origin, made by coagulating soy milk, and then pressing the resulting curds into blocks. The making of tofu from soy milk is similar to the technique of making cheese from milk. Wheat gluten, or seitan, in its steamed and fried forms, is often mistakenly called “tofu” in Asian or vegetarian dishes.

Tofu is a soy bean product.
Tofu is made by first boiling the soy beans, grind it up, strain the juice. Take the juice, add curdling agent, then cut it into squares. That's it.
You really don't make your own, but you buy it at Asian stores. Tofu itself does not have any flavor, but instead, it takes any flavor you add to it.

it's a very healthy soybean preparation that you can actually create at home if you have the time and interest. However, most stores carry a variety (ranging from extra firm to soft). The soft is better for things like burgers, while the firm is typically used in stir fry etc.
Here are some sites to get you started if you want to make it yourself:

As answered before, it's soybean curd. Some vegetarians use it as a meat substitute. By itself it has no flavor, but it takes on the flavor of whatever you cook it with. Sometimes you have to press it first to get some water out of it. Mostly you cut it into cubes, then you can stir-fry it, or bake it or whatever.
