Before Martin Luther the Catholic Church was the only church for all intents and purposes. Priests were generally polygamous until the Vatican realised that their families were costing a lot of cash to support! So in 1022 Pope Benedict VIII banned marriages and mistresses for priests and in 1139 Pope Innocent II voided all marriages of priests and all new priests had to divorce their wives.

The practice came about in the Middle Ages. I don't know the exact year. The reason behind it was simply that the church was losing too much land through priests gifting land to their children. This was a time when everyone still grew at least some of their own food, so every young couple needed a plot of land when they were starting out.

To the best of my knowledge it was about 1000 years later. You should ask jeeves, that guy knows everything. Check out this site. I dont know how reliable. Celibacy was decided on at about the same time the books of the official bible were decided on.

Depends upon which level you're talking about. Many low level clerics adopted celibacy for atonement quite early on. Popes, on the other hand, were a randy bunch until much more recently (i.e. the Borgia and Medici "dynasties" of Popes...)

Only certain parts of the church did, but I believe it was around the 400-500 time frame, whenever one of the councils of Nicea was done.
