When you see it squat, snatch it up and put it outside where you want it to pee (or on paper or wherever your designated pee spot is.) There's also some kind of spray that supposedly says, "hey, pee here!" in pheromones to puppies. You can spray that where you want the puppy to pee. Always take the puppy out regularly, especially right after it eats or drinks (cuz it might like to do other things, too.)
If that all fails, see if you can borrow a dog that is perfectly housebroken and doesn't mind puppies--a female dog might be a little more likely to discipline a puppy than a male, but take what you can get. Keep the dog around with the puppy so that the puppy sees where it's OK to go and what's off-limits. Sometimes, a dog will figure things out a lot quicker if they have a good example--I've heard of several dogs being house-trained by other dogs where they lived. Dogs often enjoy training and being trained by other dogs--it's how they do it in the wild, and if you can't teach the puppy, a dog might have a little more success. Good luck!

most dogs start whining and/or walking weird when they have to go.
Make sure you are taking your dog outside enough puppies need to go a ton more than adults. If you see your dog have an accident do not yell because it will only confuse and frighten them have a spray bottle and squirt them a little bit. If you don't like taking your dog on a leash around the house or the dogs isn't ready to go outside without one yet think about putting up a fence, but if you do I know its a big project but make sure its in the back yard or else the dog could devoulop a barking habit. I hope this helps and good luck!

You took the pup away from its family and familiar territory, but didn't bother to show it where the toilet IS. When a guest wants the bathroom, you tell them where it is, and its the same with pups.
The bad news is that this breed i one thats notorious for being slow to housetrain.
You never punish a pup for getting it wrong.
Would you leave a baby to roam your carpets and then rub its nose in it? Its insane to do that.
Here is a link to a place you can get a training booklet; you need to learn about crate training.
The crate is the pups den, and they won't mess in their own bed. They need to go at fairly regular intervals throughout the day, and you take them to the toilet and wait. You reward them for going on the right spot. They can then have a play session in the garden or house.
Or try looking it up on the Web. You should have learned how to housetrain a pup before you bought one, now that the scent is in the carpet he'll think thats the right spot to go. Plus he's learning something called 'surface preference', which means in future he'll prefer to go on the carpet rather than grass or concrete.

Wow! Nice people on here--hope they're not pet owners...
If it's a male, he's marking his territory, and since he smells it, he keeps peeing on it.
Get one of those "pee finder" lights (it's a blacklight), use it in complete darkness, find the pee. LOOK everywhere even where it doesnt smell. And then use Simple Solution, it's a "pee be gone", comes in gallon jugs, works great.
AND THEN, use the "shaker bottle" the next time you see him lifting his leg---empty vitamin bottle, put 2 pennies in it, he starts to lift, holler "NO!" and shake the bottle (or throw it at him, it wont hurt him, dont aim at him, aim next to him). And then take him out. WHen he pees outside, praise and reward.
Ok, out of breath now, hope it helps. Bichons are notoriously difficult to housebreak.

You need to be consistent with crate training her. And when you let her out then you MUST watch her for any signs of her getting ready to potty so that you can take her outside in the grass.
PetSmarts with a Banfield Vet usually has a $15 session on potty training.

Pop the dog into the garden after it wakes and after food, it should soon learn. My puppy learned in 2 days.

well lets see, if it is a boy then all boy dogs pee everywhere because they are marking their territory and other dogs that might wander in know that this house belongs to another dog and they will retreat
