Each time wen you enter the U.S., an immigration officer will write on I-94 form for how long you're admitted. I-91 is the form that will be attached to your passport. Typically it is 90 days. In the past it could be easily renewed (before 9/11), I am not sure about current situation. OK, I found it:
"Those visitors who wish to stay beyond the time indicated on their Form I-94 must contact the USCIS to request Form I-539, Application to Extend Status. The decision to grant or deny a request for extension of stay is made solely by the USCIS." Here is the link: http://www.uscis.gov/graphics/formsfee/forms/i-539.htm

Once you leave the U.S you're expected to spend at least the same amount of time in your home country (3 months in the U.S. - at least 3 months at home). Remember, that the visa does not guarantee you an automatic entry into the U.S. - the final decision is made by the immigration officer.
