i'm not really sure what the answer is... i want to know what other people's opinion are

I believe it is an IEP, Individual Education Plan.

If you are talking about just the goals and objectives that a child needs to meet, you are asking about an IEP (Individual Education Plan) or IFSP (Individual Family Service Plan). These plans are drawn up to show benchmarks that a child needs to meet in order to meet major goals. If you are referring to a plan that helps to mold behaviours of children, that lists specific goals and objectives to help them modify their behaviour, then you are asking about a Behaviour Modification Plan. A Behaviour Plan lists the antecedents to a behaviour, circumstances surrounding the behaviour, time frames when a behaviour occurs, and possible solutions to a behavior, as well as lays out a plan for changing the behaviour to something else or stopping it altogether.

A plan for a specific child's education is usually referred to as an IEP (Individual Education Plan) - I worked in the schools for over 10 years and worked with lots of special needs children and had to follow their IEP's.

I don't know what it is called, but I am familiar with the terms. We have an autistic grandson. He is being mainstreamed with a one on one person. My son and daughter in law go to school on a regular basis to review the educational and behavior goals for him. They discuss what behaviors and learnings must be reinforced in the home also.
I hope this was of help. Sorry I can't think what it is called.
