I slouch a lot while standing and when I sit down I lean foward its a habit . What can I do or buy or exercise to get a better posture and to straighten my back?

It's good that you're thinking about this, as it becomes permanent after years of it. When you are leaning forward your back is stretching, and after years of it, you will permanently lean forward, as the back gets overstretched.
At the same time, your chest is caving in, which makes it extremely tight. I would suggest two things:
1. Stretch your chest on a regular basis and open it up. Lie down on the floor and lay flat, stretch out your arms on the floor at shoulder level, allow your muscles to stretch.
2. Start working out with weights and machines to strengthen your chest and your back. By strengthen both, you will start to stand up straight, not even realizing you're doing it, as your muscles will be stronger and will hold you up automatically. The stretching will be important after you've worked these muscles.While you're at it, you may as well strengthen your shoulders, arms, and neck too!
Now that you're aware of your posture, start thinking about it and sit up straight, so no permanent damage will be done. When you're sitting or standing, pull your chin in, which will draw your head back in towards you.

You need to go for lessons in 'Alexander technique'. It's a form of exercise to realign your body. It releases tight muscles which over compensate for poor posture.
Or you could try ti chi. Yoga and Pillates also strengthen core muscles in your body which will help your posture.
There is no quick-fix, you will need to apply sustained effort to help your body overcome years of slouching, but you will be glad you did it when you feel the results.

do not slouch sit straight up and dont lean up when you sit sit back

practice. you should try stretching and focusing on your posture while you are out or sitting.
