First of all dental hygine (i.e. brushing, flossing, etc.).
Assuming that you're already doing that, there may be
several reasons why the bad breath is persisting,
and thus several things taht may work to fix it.

Firstly, there are a LOT of people who don't even
know it but have particularly pungent bacterial strains
living in their mouth... basically they have more
stinky plaque. Usually to fix this one should visit a
Dental Hygenist and get a very thourough cleaning.
Then, right away, begin brushing with toothpaste
that has Myrrh in it (I recommend Tom's of Maine brand).
(You may want to start using this even without getting a cleaning
or knowing the cause, it will still help, and Myrrh strengthens
your gums too!)
Also, be vigilant about your brushing, 3 times a day!
You may also want to get a Sulca Brush (or carry toothpicks)
to help clean between brushings, and after snacks.

Diet can be another cause of bad breath, wherein the
smell is actually rising from your stomach. In this case,
you probably need to change your diet, and I recommend
seeing a Nutritionist to help with that. You wouldn't just
be changing your diet because of the bad breath though,
chances are if your diet is what is causing it, that your
digestive system is not taking well to the foods you've been
eating, and thus bad breath is probably just the first thing wrong.
You'll want to fix your diet before you get seriously out of
balance nutritionally.

If you Smoke or drink copious amounts of Coffee those of
course can be causes of bad breath as well, and you should
try to quit.

As for other things which may help you with general bad breath...
Fresh Parsley eaten frequently is quite the breath freshener,
and also purifies your blood (nice side effect).
Or, there's always breathmints.

Flossing, mouthwash, more regular brushing, and better diet.
