If your neice had a baby what would be the correct term for that relationship? Would it be a second niece?

You would be either their Great-Uncle or Great Aunt while the baby would be either a Great-Niece or Great Nephew.

My niece has 4 daughters who range in age from 19 down to 7 yrs old....They all refer to me as their aunt, but I in reality, I believe I am a GREAT aunt....

Great-niece or grand-niece would do. Also could say niece once removed.

you would be great aunt and baby would be great niece or great nephew.Second's have to do with your parents. Your parent's cousins become your second cousins and so on.
