Yes, it is possible. If you want the picture to look natural, like your aunt was there when a family picture was taken, it's always best to find two pictures where people (in this case your aunt and the rest of your relatives) are almost the same size. Even though much can be done with Photoshop, including resizing and sharpening, if your aunt is too small, you'll have to enlarge her to make her the same size as your other relatives, and she may turn out pixelated, or viceversa. It's usually safer to reduce a picture than enlarge it, but sometimes if you reduce it too much it just won't look the same in terms of sharpness and details, even if you use adjustments, filters, etc. Also, the combination may look unnatural if one picture has a soft, diffused light and the other very harsh light and shadows. You can manipulate contrast and brightness in Photoshop, and even add shadows at that, but very few designers will be able to do a "perfect" job. Remember that there's many people out there who claim to be graphic designers or digital artists, but you never know their actual skills or degree of professionality. So, if you choose carefully the two pictures that you want to merge, the designer will be more likely to do a good job, he or she will do it faster and it will cost you less money.

With a wee bit of time on Photoshop, and an understanding of layers, masks, guides, color matching, light balancing and opacity, it can be done rather easily!

If one does not have PS skills, the fact of the matter is that you will have to pay a digital photographer, graphic artist or similar service to do it for you. It's generally not cheap because it can take a while to do.

You can do it yourself with photoshop or you would have to take it to a graphic designer however they might charge a lot. I know at colleges they have student run businesses and they are going to be the cheapest and they can do it just as good or even better then anyone else.
