Generally it doesn't cost money to enter a non-stakes race. Stakes races can have many fees (nomination, entry and starter) and the amount needed for each fee is written into the race's conditions. For example, these are the condition's for Saratoga's Amsterdam Stakes

The fee is $150 to nominate your horse (or declare him eligible for participation), $750 if you actually enter your horse in the race and an additional $750 if your horse starts in the race or a total of $1650 to run. Different races are run under different conditions and the fees vary from race to race.

It depends on all of the above things you listed.
There are nomination fees and then actual entry fees if you run the horse.
Each track has various set fees of things and it depends on the type of race and quality of the race.
It is expensive to have a horse race. By the time you pay just the stable rate and pony fees and feed and such you have a fair ammount invested and then you have the entry fees as well.
