It is obvious to me that you are his best present. Relax and be yourself.
Let him choose. He probably has some ideas of people and places he'd like to see before he dies. Your time, interest and love are the best things you can give him. Make him comfortable and clean, and he'll really appreciate it. Make your place cheery. Ask what kind of music he likes. Perhaps, if he has friends in your area, you can invite them over for visits.
I don't know if he can eat or not. If there isn't a problem, here, throw a barbecue or a picnic.
Do you have a small pet that he could love? If not, maybe your friends could drop by, occasionally, with their pets.
Listen to his stories and laugh even if he repeats himself.
I lost my uncle in the springtime. He was my last contact with his generation. I really miss him.
You must really love your uncle to take on such a huge task. This will leave you with good memories and a smile in your heart.

This is a confusing question, I am glad you asked a master answerer like me. The answer is you should get him a tin of beans and a beverage, since he will be tired from the trip. You can consider this a present, although many would call it an OFFERING.

He really just wants to spend time with you.
If he is up to it, take him to some nice areas
that you'll have as a memory, don't forget
photos. Do you have a digital camera?
Than give him a copy of the treasured event.
Have a lovely time with your uncle.

I believe that a present is unnecessary. If you give him your attention when he is speaking, if you pay attention to his needs and wants, if you give him love and kindness, you will have given
some wonderful gifts. Show him respect and please make sure that his needs, and there may be many, are met.

I would suggest please see some of the 106 issues of "INSPIRE" available in the undernoted website. All may not impress you essentially but some of these would surely show you something which you can gift to your uncle. Click "Watch a video" in the various issues of INSPIRE which you would see in the website.

If it really helps you and him please let me know.
