You should get your mom something thoughtful, and definitely include a card. Depdending on your budget, you can make something for her, take her to dinner or somewhere that she'd like, or buy her something you know she'd want but would never buy for herself. No matter what you get her, it will be special because it will be from you :)

Do you write poetry or can you tell her how much you appreciate her in your own words? Something like this is so personal and she would probably love it. Also, a trip to a day spa to get a massage or a facial or her nails done, something she doesn't normally get a chance to do! With all the responses you will get, I'm sure you will find the perfect gift for your mom!

A pretty bouquet of flowers and a card. If you know something she collects, get her something in that genre. My mom collects bird houses and loves to cook, so I got her a bird house recipe holder. Good luck!

spend the day doing anything with her (shopping, dinner, get your nails done or hair) then treat her to something that she wouldn't normally buy for herself like those incredibly expensive fuzzy house shoes that feel so good in the winter.
