All answers so far are wrong. Let the water run cold at the tap for at least 1 minute (you want it to get oxygenated) before filling your tea kettle. Bring to a slow boil, not a rolling boil. Remove from heat. Pour boiling water into your teapot or mug and swirl around. Pour off that water. Place 2 teaspoons tea leaves per 6 ounces of water in a paper filter(available at tea and coffee shops). Pour the boiling water OVER the leaves in the filter. Let steep 3-5 minutes. Remove the filter(also known as "decanting"). Then add sugar (if you must) followed by milk. Never mix milk and lemon or the milk will curdle.
Milk is always added after the tea has been poured (unless you are making Chai). You want to be able to judge the strength of the tea before adding anything to it.
Leaving the leaves in the pot will cause the tea to turn bitter.

The way I was told to prepare tea (China, Japan and Sri Lanka) is as follows:
Boil water, then let it cool off a bit (few degrees), add tea leaves. Let it brew for approx 5 min or according to your taste. Orientals do not add sugars or milk ; usually I add sugar and a slice of lemon (this is what the italian call blond tea because the color lightens)
I used to boil water and add tea leaves or tea bag, was was told NOT to that because you don't want cook the tea leaves, only to enhance the flavor, and that is done when the water is either at room temperature (leave tea overnight) or hotter, not more than 95 degrees F or 35 degrees C .

Boil the water and let it cool a few minutes. The ideal brewing temperature is 80 celsius, but who really measures. Brew the tea and then add the sugar and milk before you drink it.
Don't boil the tea leaves, it makes them bitter.
