Transcription/translation errors when DNA is being read and duplicated. Also chemical mutation from different types of radiation (not just the nuclear kind). Mostly the former rather than the latter.

I would say it is mutations in the DNA but it would have to be in cells that are passed down to the offspring (egg and sperm cells) because evolution occurs over many generations.

what prompts it... maybe during dna replication in the cell cycle before cells undergo meiosis?
