You don't say whether you are the insured or just a claimant. If you are an insured (it's your company) then they have a duty (per your policy and/or by law) to respond quickly.

If you are just a claimant (it is not your company) they don't owe you the same duty so how they respond is not govern by any laws. I would expect 30 days and if they respond sooner then it will probably be an offer you don't want to consider to begin with. In fact, I would never accept the first offer unless it is for the policy limits in which case that is probably the extent of what you would receive. You could sue for more but you have to decide whether there is any chance of you collecting and what the attorney's fees and collection cost are going to amount to.

As a rule of thumb, I would say 10 business days. At this point the insurance company should have all the information they need to evaluate the claim. Depending on the amount being asked for in the demand, the adjuster may be able to settle the matter on their own authority or for large claims they may have to send it up the chain. But some type of response should be made within the time set forth.

Days to weeks. Depending if the adjuster gets off their tail and dies their job.
