what's a good layout that's simple for a custom t-shirt site?

You mean like a CafePress shop? There are templates and plenty of help modifing them in the CafePress forums. It would be hard to get more specific without knowing what kinds of shirts you are doing. Children shirts, biker shirts, gothic?
If its going to be a general do-anything type of shop then you should be aware that is the hardest to do. Its harder to get traffic to a do-anything site than it is to catch the attention of a small niche.
