Ive had several removed...
I got a referral by my PCP (primary care provider) to a dermatologist. All I had to pay was my co-pays and my insurance covered the rest.
I've had the small in office ones removed and had three larger ones removed that required a short stay in the hospital. Again all covered and I have crappy Aetna insurance.
I'm sure the could do one of the ear but you might have to find the right Dr who would. I guess it depends on the size, depth and where at on your ear it is located.
PS Watch Aetna they don't like to pay things. I got a notification from them saying my mole removal was cosmetic surgery.....dumb*sses didn't see the part on there about the MELANOMA. Usually the dermatologist can decide if it is cosmetic or something that may turn cancerous. Most are just cautious and remove them anyway. Just make sure they file it properly under necessary and not cosmetic.
Insurance sucks.

It depends on the size of the mole and what your doctor's fees are.
Insurence doesn't usually cover it unless it's medically necessary to remove it.
Yes, one can be removed from the ear, but only if it's on the outter ear. There may be a scar or part of your ear missing afterwards, but usually not enough for anyone to notice.

If you doctor think it might look bad, like cancerous, which most will say, insurance covers it and the procedure is usually done right there in the office. As for the ear, I don't see why not. Good luck and ask your doc about it.

if a certain size or color ( MD will know) mole removal can be covered by insurance
IF only for appearance sake? just remember the scar left behind might be just as noticeable as the mole was

Insurance won't cover it and prices can range between 25 bucks to 350 depending on the method, its important to know that the hyperpigmentation can come back so you might have to go through the process again

It's expensive, insurance should cover it (mine did), and I am sure they could remove one that's on the ear.

Call you doctor's office and ask, and then ask your insurance carrier what there plan covers--it varies. But, it's a good preventive move, they may go for it.
