Easy..

London: Arsenal, Chelsea, Tottenham Hotspur, Fulham, Charlton Athletic, West Ham United. (Watford is situated just north of the boundary of Greater London, but is very close)

Manchester: Manchester United, Manchester City ( Bolton Wanderers is situated in Bolton, very close to Manchester)

Liverpool: Liverpool, Everton

Birmingham: Aston Villa

All the rest are in the town or city whose name they carry. Wigan Athletic is in Lancashire and not far from Manchester, Middlesborough and Newcastle are situated in the north-east corner of England, Portsmouth is on the south coast, Sheffield United is in south Yorkshire, and Reading is situated about 30 miles west of London

Think thats the lot...and a map of England should be easy to find.

i dont know where you can find a map but if you go to wikipedia and type in the name of the club, it will tell you not only the city where they are based, but info on the club, as well as a list of its players (old, current, and new).
