Mostly, because boys are afraid of how to handle rejection. They visualize the situation over and over and the thought of her saying, "no" is too frightening.
Girls are often together in groups, which makes it even more difficult to approach her. The poor guy would have to face the whole pack of them and overcome the fear of being rejected in front of them all.
Eventually, boys learn that rejection isn't so bad and they start asking lots of girls out. But the first few are very difficult.

Because girls act like they are not interested.
Because the boy wants the girl very much and it is devastating to get turned down.
Because girls have many excuses like "I don't know you well enough", I'm not dating now, my dog is sick, I'm kind of already seeing someone, you are too old, I have a boyfriend, etc etc".
Because dating is a big time status game, and guys think they are not worthy of a girl's attention and she does everything she can to make it seem that way. Girls are wanting to go for that "special guy". Girls/women believe that "sex is dirty". The guy cannot imagine how a girl could want or allow him to do "that" to her. Girls reject guys a lot, basically tell them to shove off, you're not good enough for me. It is a lot easier to just go home and imagine you are with the girl and masturbate. Some guys masturbate so much that their penis becomes raw and sore.

Because you scare us back into our world of doubt every time. You not only say no, you say it in a way as to make usfeel as though we never had a chance in hell. The truth is, women only want the guys that they want to ask them out. It's almost like they are affended when a guy asks that they are not interested in. It seems women reject harshly, as if the guy won't get it. If your going to reject, reject with niceness and respect and that way our egos won't be so kicked around. Then, we can recover quicker and gain confidence to keep asking you girls out.

Because beautiful girls have more attitude and that's the reason why boys feel scared to ask girls or directly make friendship with them.

How come you are able to read my mind? Hmm.. let me think.. rejection (just anyone says) definitely the biggest factor, but... the status of the relationship after that also plays important factor. Say.. I like someone, and she doesn't want to go out with me. Eventhough it is competely fine with me, but she may act differently in the future (stop talking, staying away, etc). This is the part that I still don't get it until now :( You may explain me about this part next time.

I myself am terrified to ask any girl out, wether she was hot or average... because im afraid i might get no as an answer and the impact of that scenario on me

I don't think it is because of the fear of rejection. I think it is because with a YES, the boys have to figure out what to do next.

fear of rejection. they don't know how to ask a girl out. two reasons i can come up with. they have not yet developed that skill of men who play with the emotions of women. oh women have that already, even when they are still young.
