Hi. As question says, what can someone with social anxiety who has failed in attempts to interact in small groups do. Particularly if medication isn't an option given past ineffectiveness and therapist fears of overdose attempt of prescribed?

If you are dealing with social anxiety and attempts to interact in small groups fail, you must change your approach. Don't let the GROUP overwhelm you.

When in a group, concentrate on ONE person. Pay ONLY attention to that one person.
You do not have to react to the others all at once.

When that ONE person inevitably talks to a second person, try to talk to THAT person just a little bit too, or at least stay TUNED into the discussion, and listen to them talk. You will find that you will be able to contribute to that discussion in time.

Eventually you shall be able to handle several people at once.

I suggest contacting a nursing home and requesting short visits with one resident. You may be far less anxious if you are offering something of value to someone else. With this disorder, one-on-one is a good place to start, then perhaps you can add a second person to the "group." If this does not appeal to you, look for volunteer opportunities in some area where you have an interest. The more you interact, the less anxious you will be. In this case, even without meds, success really breeds success. Good luck

Pets and trained support animals can be great for helping people with social anxiety. The attention of the group becomes focused on the animal instead of the anxious person and immediately creates a relaxed atmosphere.
