You need to phone t-mobile customer services (if you have thrown out the stuff you got with the phone) - n.o should be on their website. They are able to give you it. don't keep entering it incorrectly or you will block the SIM.

you can only obtain a PUK code by contacting customer service, its a special number formed from you SIM number and you cells IMEI number.

Speak to your service provider, they will have it on their system. If the phone is a knock off then I would get a legitimate phone on pay as you talk or a monthly contract.
