another reason is hormonal imbalance, and now that requires more investigations, she has to see a doctor if there are other symptoms. such as, hypertension, Buffalo hump (that's fat collection in the back in the inter-scapular area) menstrual distrubances, acne, hirsutism, skin pigmentation and others.. she might be having what's called Cushing's Syndrome.
a third and most common reason might be genetic factor. she might be absolutely normal that way. check her parents and her family.

if it is "moon" shaped have her check with a doc. especially if it has started happening with in a few months to the last year.
might be nothing or it could be thyroid related.
