I think you mean the Amish.

The Amish avoid the use of machines requiring electricity. They live in close-knit farming communities and travel by horse and buggy. They do use manual (animal-drawn) plows and the wheel. I know that they do not believe in using health insurance, but I have never heard that they avoid hospitals or medicine. They are commonly found in Pennsylvania, although not exclusively.

The Mennonites are closely related to the Amish, but are not the same. They wear more traditional 18'th century clothing, but most will use automobiles. Some Old Order Mennonites will not use automobiles or tractors for farming.

The Quakers are not the same as Amish, although they are also commonly found in Pennsylvania. Quakers do not have the same prohibition on the use of technology.

All three of these groups emphasize plain dress and lack of adornment. Quakers tend to dress conservatively, but don't tend to wear the more "period" dress that the Amish and Mennonite do.

As for caffeine and chocolate - I think that might refer to Mormons (Church of Latter-Day Saints), whom I have been told avoid stimulating or altering substances like caffeine (in coffee, soda, chocolate) or smoking cigarettes/using tobacco because it impairs the purity of their body.

My daughter has an incurable illness that continually has us at the hospitals. Over the past 12 years I have seen people from many cultures as the hospitals she goes to are world renowned. I've seen Amish there many times and with their children. We ourselves are Jehovah's Witnesses, and we seek the best medical care we can find. So those two faiths can not be included.

Amish....Yeah...I saw a documentary on them...it was showing how they let their kids go out into the 'real world' when they turn 16 and after spending some years living how most of us do...drinking and partying and smoking etc....they then by the age of 21 must make a final decision where they want to be. Most chose to go back. It really made you see how screwed up 'our' world is and how they know how to live properly.
Where's my bourbon!!! ;-)

Amish. But different sects have different limitations.

Oh, and Quakers are not the same as the Amish.

amish. not mennonite, since the use cars, and shop at grocery stores (you may see them sometimes), but the amish are very, um, old-school. while mennonites are an offshoot of this same religion, the strict beliefs are not the same.
