You need to approach the question with tact, but directly. Directly is the key here. You can say, "I have noticed that people are sometimes offended when you do not button your shirt all the way up. My understanding is that proper business attire is a completely buttoned shirt, and I am afraid you look unprofessional when you do not do this. For my sake, could you please make this standard practice?"
In reality, your supervisor should be the one addressing this problem, not you. You may want to mention it to him/her.
This is an assertive, non-judgmental way to deal with problems.

If you're friendly, and he's friendly, reach up and button it for him, he'll think you're being playful but really you'll be saving the whole office from his daily horror show. Then tell him playfully that's how you expect to see his shirt everyday and laugh. But know what kind of person he is and what type of mood he's in because everyone doesn't take too kindly to jokes and being touched either.

Keep mentioning to him his button is undone like you are helping him out, as if he had something on his face or shirt. Be polite but every time you see him mention it.

If it's a co-worker then I would mention it privately to the boss. Then he/she can address the problem in a respectful yet authoritative manner.
