Clean the sub floor and put down a coat of Kilz or Bullseye sealer before installing the new flooring. I would also open all the windows for a day or two to air out the house, that will do wonders for the smell. This is the same sealer we use on burnt houses to stop the smoke smell, it will work for mold and mildew as well.

Take 5 litres water mixed with 300 ml of Coca Cola. It will clean the floor as good as bleach. Trucker use Coca cola 1 litre bottle to clean there greasy truck engines.

The only way to get rid of it permantly is to find a company that uses ozone generators. Bleach can only reach surfaces that are open. Ozone generators can penetrate into places where chemicals can`t get to. Use an ammonia cleaner to get rid of the dog smell.

Scrub everything with bleach, and spread sodium Bicarb around. If there are any places that sell livestock feeds close to you you can buy 50 pound bags for cheap.
