It depends on how she feels about you & how much exciting conversation you were having prior to asking her for the number. If she is turned off, it means she is not into you, or that you were too desperate.
Mrs. M, calm down - what if he was a good guy and both of them liked each other a lot? If he was bad, then she should be adult enough to find that out.

If you don't there may never be a second encounter, however make sure there are signs there, she is smiling, checking your out things like that, just walking up to random women and asking for their numbers could probably get you in trouble.

If it is, that's stupid - I mean, how else are you going to see her again?
Anyway, in this day and age I don't think it is; but, I'm not a girl, nor have I ever actually asked for a girl's phone number.

Depends on the situation to be honest. If you know you will see her again it can't hurt to try and get to know her more before trying to get the number.
