Currently, it could depend on whether you are a legal adult of sound mind or not.

There is a case of a 16-yr-old boy who is refusing chemotherapy for lymphoma. Previously, the courts ruled that he is not old enough to make an informed decision about his course of treatment, so he became a ward of the court and was going to be forced to take treatment. But then a judge overturned that ruling.

It's a tough call -- are you so ideologically libertarian that you would allow a boy to die from choosing quackery to prove your point?

If you are a sane adult, yes, you are able to determine your course of treatment provided your doctor explains your options to you. If you are not mentally competent (crazy or comatose or otherwise), then you cannot decide for yourself and someone else (usually next of kin) will decide it for you.

The important thing is to do your research. Listen to your doctor. Be informed about your options and don't be swayed by snake-oil scammers.

Doctors or your appointed guardian can make decisions about your health care regardless of your consent if:

you are declared legally incompetent due to drug addiction or severe mental illness and you are a danger to yourself or someone else.

you are a minor and cannot give consent.

Other than that, the choice is yours.

In some situations, they can obtain an order to keep you in the hospital for treatment, but they must file a motion and have it heard by a judge.

You have the choice. My aunt just found out she's in the final stages of cancer and the docs gave her the choice of going through chemotherapy to try to fight it or just let the cancer take its course. She's fighting it.
