I need to fill out a financial aid application for my child and it asks for my health insurance policy number. My card doesn't list a policy number. It only has:

Subscriber number
Group number
and Card number

WHICH one is the policy number?! Thanks in advance!

Your subscriber # is your policy #. (Don't listen to dude #1)

It is the group # as well as the subscriber #

The group is the first part of the policy #
the subscriber # is the second part of the policy number #

It is kind of like an address, where you have a town and then the street.

i believe on that particular insurance the policy number contains the holder's ss # but to be sure you could always call the 800 number on the card and ask them
