When you're speaking with him, call him Mr. Smith or Bob, if you're on a first name basis with him.
When writing a letter, address it to Bob Smith, Esq., and in the salutation line, type Dear Atty. Smith.

There are a few WRITTEN forms of address that I'm aware of, such as Esquire, or Attorney at Law, or Counselor. But in a verbal address, I don't know there's any common form. If I had to pick one, I'd say Counselor.

Write the address to where he is going on his back, and put the stamp above his right eye. Post office will probably charge extra for the weight.
