2) Accidents ARE something bad that happens.
3) An accident IS something bad that happens.
"Accident" is a noun, but it acts as a preposition. Therefore you must take into consideration it's tense. Past, Present or Future.
If Future and plural you must use verb "are" along with the intr. v. "Happens."

I think either one will do but you could just avoid the issue and call it an inadvertent occurrence with a generally negative connotation.
