I've never heard of peanuts in Dr Pepper, but I'm originally from Atlanta, Georgia, and we used to put a handful of roasted salted peanuts in our glass bottles of ice cold Coca-Cola mmmmmm

When I was a kid, my parents always did this with Coca-cola, but I don't remember them doing it with Dr. Pepper or not. I still do it sometimes. It just gives the peanuts a different flavor.

It's just a custom; a tradition.
Different areas or ethnicitys of people do things that are particular to that group or area.
such as:
Boston Baked Beans.
Creole Gumbo.
Clam Bakes..
Roast Suckling Pig.
Chinese Stir Fry.
Look around you; all things have their beginnings from a group or area.
Custom.

It used to be a thing when I was in high school in the 90's. I never understood it though. I've always avoided drinks with floaters.

Sounds gross to me. Have you tried it? I cant promise I will rush to. But to each his own and for those who do like it (wherever those weirdos are) go for the gold.
