Infectious Mononucleosis is fairly rare on children that age the age of unset is usually between 10 and 35 years and since i am no pediatrician you should talk to him/her to find out who often these complications are.
Since there normally is Splenomegaly which could also lead to Hepatomegaly this is one issue your doctor should direct his/her attention to, but since you mention recover then I'll limit to mention some of the complication.
Most complication of EB virus are rare but they include throat infection (secondary) pericarditis and myocarditis, Splenic rupture is another situation that might be seen and least Neurologic involvement such as transverse myelitis and encephalitis, but as i said before these are extremely rare

Wow. THREE? Thats awefully young.
Not surprisingly, vaccines can cause an overload sometimes and it's result is mono. Have seen this after being innoculated by Varicella vaccines as well as just too many in a short period of time (as with all children).
I would have some tests run to rule out disease of the spleen, pancreas and the liver. I say this JUST to be safe as Mono is relative to HCV- Flafiviradae viruses. I believe they have ruled out the connection, but it wont hurt to have him get tested!
Please also, being three year old means a lot of activity...Too much could be a danger as the spleen can become inflammed and burst. (stomache pain).
Not to alarm you however, I am sure he will be just fine.

His hurting tummy is his liver ( it swells), wich is normal when you have a mono. I've had it, I know people who had it and never heard of complications. Go see your doctor to make sure everything is all right with you're little kiddo.
