Actually it is a very easy process to tell you the truth. They pluck the fur off the scrotum with tweezers because for the cat this is a lot better than trying to shave such a tiny area and the fur grows back quickly and isn't itchy or anything like that. Once the fur is plucked an incision is made length wise which isn't even long enough that requires stitches afterwards. That's right it is that small. They the testicles are sort of popped out and they are cut out leaving the veins long enough that you can tie them in knots. As many knots are tied as possible. That's it, that's all. Sounds awful but for a cat they really don't even notice.

It depends on the doctor. My mom's old vet used to remove the entire sac, but her new vet only removes the testicles.
If you are in need of knowing if a cat has been neutered or not, if the testicles look flat (like there's no contents to them), then he's been neutered. If worse comes to worse, try feeling them...

Just the testicles. The scrotum stays- there's no reason to remove that too, it would just cause a larger surgical wound.

I have had 5 surgeries in my life so far, only 1 for cosmetic reasons. There are SO MANY things, preparations, testing that has to go on before you can have surgery. Many of these are precautions doctors take so that you know if your body can handle having a surgery in the first place. They also have blood on hand to save your life if something goes wrong. So even if you are ready next comes prep and sterilization of rooms, areas, equipment, tools & u. This is going to be nearly impossible for u to accomplish at home by yourself. You are going to have a MAJOR open wound, that even if you make it through your home procedure, that doesn't mean that days from now that u may not die from infection. You probably will. Also, doctors never perform a procedure alone, they always have the help of a team or at least one other person like a nurse. That someone or team are usually the people that are monitoring your breathing & helping to stop excessive blood loss while both of your surgeons hands are busy cutting, fixing etc. If u want an honest answer to your question, do it right & go to a real surgeon. I know your urgency is based on u not wanting to become "big" but the limited "knowledge" I have is when I watched the TV Show that had Cher's daughter Chastity Bono documenting her journey to become a man. I know that it is mostly a person's hormones that determine femininity or masculinity so maybe it would be possible for u to go consult with a Dr. to start some form of hormone treatments until u can save up enough money to get the surgery. I would also inquire with the doctor what natural ways u may be able to up your estrogen levels & only do them if he deems them safe. I know menopausal women lose estrogen & some will eat sweet potatoes to help them with balancing or replacing the estrogen that is lost. Without that us women end up with areas sometimes that grow hair like men do to a degree (facial for example) so for men a lack of testosterone would be the opposite..you get more girly in a sense. Before I go, I think the doctor consult and hormone treatments are worth a look at as a start. Be completely honest with the doctor & don't hold anything back. Don't be afraid of shocking them or being ashamed etc. They are in the business & will be the ones to shock you :) If after reading this you still have any traces in your mind that you want to go through with this, go look up your surgery on YouTube to see if it is on there, it probably is (all of mine were) & after watching it I guarantee you that you will be phoning a real plastic surgeon in the morning! btw most girls like penis & the balls are an added bonus so you know... & most modified parts never have a normal feeling sensation again especially if you sever the nerves or have scar tissue, it took my nose almost 5 years to feel like it did before my surgery.) They remove the testicles and the outer scrotum should shrink away according to my vet.

My vet did the whole thing, but other vets just take the testicles out.
Oh, and you can get your cat Neuticles, so the poor things still have something to lick.

The testes are removed through a small incision in the scrotum.
The scrotum remains and will have a couple stitches where the incision was made.
